Job Description Summary

The Community Outreach and Relations Consultant plays a key role in advancing CareMore Health's mission to deliver whole-person, value-based care by developing and executing community engagement strategies that strengthen relationships with members, community-based organizations, healthcare providers, advocacy groups, public agencies, and local stakeholders. This role serves as a trusted ambassador for CareMore Health, building strategic partnerships that improve healthcare access, advance health equity, and support preventive care and population health initiatives across assigned markets.
Working independently, the Community Outreach and Relations Consultant applies advanced judgment to develop outreach strategies, cultivate community partnerships, identify emerging community needs, and provide strategic guidance to internal stakeholders. By translating community insights into actionable recommendations, this role helps ensure CareMore Health's programs, services, and engagement efforts remain responsive to local populations while supporting organizational growth, member engagement, and quality outcomes.

Key Responsibilities

  • Develop and execute comprehensive community outreach and engagement strategies that support CareMore Health's value-based care model, population health initiatives, and growth objectives across assigned markets and communities.
  • Build, cultivate, and sustain strategic relationships with community-based organizations, federally qualified health centers (FQHCs), faith-based organizations, senior centers, advocacy groups, social service agencies, public health departments, provider organizations, and civic leaders.
  • Represent CareMore Health at community events, health fairs, educational forums, coalition meetings, and stakeholder engagements to promote awareness of CareMore Health's programs, preventive services, care management resources, and commitment to whole-person care.
  • Analyze community health trends, partner feedback, member insights, demographic data, social determinants of health (SDOH), and engagement outcomes to identify opportunities that improve healthcare access, member experience, and community impact.
  • Partner with Marketing, Clinical Operations, Population Health, Care Management, Provider Relations, Network Management, Growth, Member Services, and Quality teams to align community engagement strategies with organizational priorities and local market needs.
  • Develop outreach plans, community partnership strategies, educational presentations, communication materials, stakeholder briefings, and event strategies that support consistent, culturally responsive community engagement.
  • Identify and cultivate partnerships that address barriers to care, including transportation, food insecurity, housing instability, behavioral health, and other social drivers of health affecting CareMore Health members.
  • Serve as a strategic advisor to internal leadership by providing insights on community dynamics, partnership opportunities, health equity initiatives, local market conditions, and emerging engagement risks.
  • Monitor outreach performance, partnership effectiveness, referral activity, and community engagement metrics, recommending improvements that strengthen program effectiveness and member participation.
  • Ensure outreach activities comply with organizational policies, regulatory requirements, privacy standards, and culturally competent engagement practices while reinforcing CareMore Health's mission and values.

Qualifications

  • Bachelor's degree required; Master's degree in Public Health, Healthcare Administration, Communications, Social Work, or a related field is preferred.
  • 4–6 years of experience in community relations, community outreach, public health, healthcare marketing, population health, nonprofit partnerships, healthcare operations, or a related field.
  • Advanced knowledge of community engagement strategies, stakeholder relationship management, partnership development, health equity, and public-facing healthcare outreach.
  • Experience supporting value-based care, Medicare Advantage, Medicaid, managed care, care coordination, or population health initiatives is strongly preferred.
  • Demonstrated success independently managing community engagement programs, building strategic partnerships, and influencing stakeholders across diverse organizations without direct authority.
  • Experience interpreting community health data, engagement metrics, member feedback, market trends, and partner insights to develop recommendations that improve outreach effectiveness and healthcare access.
  • Proficiency with Microsoft Office Suite, CRM and stakeholder relationship management platforms, presentation software, event management systems, and reporting tools.
  • Excellent relationship-building, facilitation, public speaking, presentation, written communication, and cross-functional collaboration skills.
  • Strong understanding of social determinants of health (SDOH), culturally responsive engagement, and strategies to reduce health disparities across diverse communities.
  • Demonstrated commitment to improving health outcomes through community partnerships, preventive care, member engagement, and whole-person care.
  • Bilingual language skills are preferred based on market and community needs.

Compensation Range:

$67,349.00

to

$101,023.00


The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
