Full Time/Part Time/ PRN Home Health Nurse RN/LPN

Can earn 65k-100k or even more based on weekly points that fit your needs. We weigh Start of Care higher, include non-visit care management points each week, and additional points for high tech visits. Typical work week is Mon - Fri with opportunities to see patients on the weekends.

Job Description
Registered Nurse - Overview:

• OASIS Start of Care and case management
• Assessing or re-evaluating the patient's condition and needs to determine plan of care
• Provide patient care according to patient's care plan and needs
• Teach the patient and/or family members about care and support skills
• Medication management and education for patient and/or family
• Communicate regularly with Clinical Care Coordinators, Interdisciplinary team, and overseeing physician
• Accurately and thoroughly document patient care within company policy
• Supervision of License Practical Nurse
• Develop care plan and supervision of home health aid when applicable
• Be an advocate for your patients and their families as you provide the tools and training to allow them to maintain a safe quality of life

Qualifications:

• Licensed as a Registered Nurse or LPN in the state of Illinois with good standings
• Valid Driver's License and reliable vehicle with current insurance
• Current CPR Certification
• Prior Nursing experience in a Home Health, Hospice, Rehab, or Hospital setting is required
• Experience with Electronic Medical Records (EMR) software in home health
• Comfortable with technology - a company tablet will be provided
• Strong communication and interpersonal skills
• Time management to balance patient care, travel, and documentation
• Great customer service
