Job Summary
We are seeking compassionate and dedicated End of Life Caregivers to provide exceptional support and comfort to patients in their final stages of life. This role involves assisting with daily activities, monitoring patient well-being, and ensuring a respectful and dignified environment. Ideal candidates will have experience in senior care, hospice, or home health settings, with a strong focus on patient-centered care and emotional support. Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and document patient vital signs and overall condition
- Provide medication administration as prescribed, adhering to HIPAA and safety protocols
- Support with meal preparation and feeding, ensuring nutritional needs are met
- Utilize specialized equipment such as Hoyer lifts or resident lifts to safely transfer patients
- Observe and report changes in patient condition or behavior to healthcare professionals
- Maintain a clean and safe environment through regular cleaning and sanitation procedures
- Follow individualized care plans tailored to each patient’s needs, including dementia, Alzheimer's, or memory care requirements
- Assist with hospice or end-of-life comfort measures, providing emotional support to patients and families
- Comply with all healthcare regulations and privacy standards, including HIPAA guidelines
Requirements
- Prior experience in home health, hospice care, assisted living, nursing homes, or long-term care settings preferred
- Knowledge of patient observation, medication administration, and vital signs monitoring
- Certifications such as CPR and First Aid are highly desirable
- Experience working with individuals with developmental disabilities, autism, dementia, or memory care needs is a plus
- Ability to perform heavy lifting safely using appropriate equipment (e.g., Hoyer lift) when necessary
- Strong communication skills and compassionate demeanor when working with patients and families
- Ability to work independently while following care plans accurately
- Valid driver’s license preferred for transportation purposes if required by the role
- Commitment to maintaining patient confidentiality in accordance with HIPAA regulations
- Flexibility to work various shifts as needed; some positions may require availability for overnight or weekend shifts depending on the facility’s needs
