CareGivers America – NEPA, a Modivcare Home Care company, is seeking a skilled and mission-driven Registered Nurse with a passion for pediatrics to join our Home Health team as an RN Case Manager.

Work Location: On the Road (Branch Location: 5 South Market Street, Suite 30, Selinsgrove, PA 17870)
Territory: Covering primarily parts of Schuykill, West End Berks, North Umberland, Snyder counties.
Schedule: Full-time | 40 hours per week
Monday–Sunday | 11:00 AM – 8:00 PM (5 days per week, including weekdays and weekends)

As a Pediatric RN Case Manager, you will:

• Complete comprehensive patient assessments, including OASIS data collection, within established timelines
• Collaborate with physicians, other healthcare professionals, patients, and families to develop individualized pediatric care plans
• Provide direction and supervision to LPNs and Home Health Aides, ensuring quality and effective care delivery for children
• Monitor clinical documentation for accuracy, timeliness, and compliance with all regulatory standards
• Communicate regularly with pediatric patients' families and care teams to coordinate services and address changing needs
• Participate in quality improvement initiatives and staff education
• Promote a safe and respectful care environment for patients and colleagues

We Are Excited to Speak to Someone With…

• Active PA RN License (Required) – BSN Preferred
• 2 years of professional nursing experience (pediatric or home health experience preferred)
• Strong leadership, communication, and organizational skills
• Ability to manage multiple priorities and work independently
• Valid driver's license, and current CPR certification
