Per Diem Pay Highlights
- OASIS Start of Care (SOC): $120
- OASIS Resumption of Care / Recertification: $100
- Routine / Follow-Up Visits: $60
- Mileage reimbursement provided
As a Pediatric RN Case Manager, you will:
- Complete comprehensive patient assessments, including OASIS data collection, within established timelines
- Collaborate with physicians, healthcare professionals, patients, and families to develop individualized pediatric care plans
- Provide direction and supervision to LPNs and Home Health Aides to ensure quality care
- Monitor clinical documentation for accuracy, timeliness, and regulatory compliance
- Communicate regularly with patients’ families and care teams to coordinate services
- Participate in quality improvement initiatives and staff education
- Promote a safe and respectful care environment for patients and colleagues
- Active PA RN License (Required) – BSN Preferred
- 2 years of professional nursing experience (pediatric or home health experience preferred)
- Strong leadership, communication, and organizational skills
- Ability to manage multiple priorities and work independently
- Valid driver’s license, and current CPR certification
