Schedule & Compensation
Hours: Monday–Friday, daytime schedule
Pay: Based on experience

What You'll Do
As the LPN Care Coordinator, you'll play an important part in ensuring residents receive exceptional care each day while working within a supportive interdisciplinary team. Your responsibilities include:

Providing hands-on nursing care including medication administration, treatments, and ADL support
Working closely with the RN and team leads to support individualized care plans
Completing documentation with accuracy and attention to detail
Supporting Resident Assistants through mentorship, coaching, and positive leadership
Managing medication orders, refills, and overall medication processes
Performing supervisory visits, completing chart reviews, and ensuring compliance with company policies and our Medicare Comprehensive License

What We're Looking For
Current Minnesota LPN license
Strong communication skills with the ability to work professionally and respectfully with staff, residents, and families
Experience in Assisted Living highly preferred
Familiarity with Alzheimer's/dementia care is a plus
Confidence using electronic documentation systems
A team-oriented mindset and desire to contribute to a supportive work culture
