SCHEDULE: Full-time position available.
PRIMARY FOCUS OF THE ROLE
The Case Manager Coordinator's primary responsibility is to manage all relationships with external case managers as they relate to residents on Elderly Waiver, CADI Waiver, and Brain Injury Waivers. The ideal candidate will have prior knowledge of the Medicaid system within assisted living settings. In addition, the role includes traditional social work responsibilities outlined below. Travel to other Carefree Living locations may be required based on the needs of the business.
KEY RESPONSIBILITIES

Assess and develop care plans that meet residents' needs. Provide or arrange social services for residents and families as appropriate.

Complete all social work–related documentation. Share information, observe and report resident conditions, and educate families and residents regarding placement, care, and rights.

Interview residents and/or families prior to admission to ensure all admission forms are completed.

Collaborate with external psychologists, psychiatrists, and providers to integrate their recommendations into the resident's care plan to support psychosocial needs.

Promote and monitor the implementation of the Resident Bill of Rights and serve as the resident's advocate.

Support residents and families with emotional needs, including acceptance of loss, relationship changes, and financial adjustments.

Ensure social service interventions are thoroughly documented and appropriate actions are taken.

Assist residents with problem solving and completing resident satisfaction surveys in accordance with facility policy.

Review resident charts and contact families to gather background information to develop social histories for new admissions.

Serve as liaison between residents, families, the facility, and the community.

Assist with discharge planning for residents transferring or returning to the community.

Provide information on advance directives and engage in related discussions as needed.

Assist residents and families with financial and business-related matters in coordination with the Business Office and Admissions Department.

Attend staffing meetings and care plan conferences and assist in developing resident goals.

Maintain records of all social work contacts with families, friends, and community agencies.

Participate in continuous quality improvement (QAPI) efforts and serve on task forces and committees.

Organize and facilitate family support groups at regular intervals.

Facilitate psychosocial groups and one-on-one interactions.
