Overview
We are seeking a compassionate and dedicated Home Care Aide to provide exceptional in-home support to individuals requiring assistance with daily activities. Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support.
- Monitor patient health by observing vital signs and reporting changes to healthcare professionals promptly.
- Administer medications accurately following prescribed instructions while maintaining detailed medical documentation.
- Support clients with specialized care needs such as Alzheimer’s disease, dementia, or memory care requirements.
- Operate medical equipment such as Hoyer lifts, catheter care devices, and EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems for documentation.
- Prepare nutritious meals tailored to dietary restrictions and assist with meal planning and preparation.
- Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment to promote overall health and safety.
- Collaborate with healthcare teams to ensure comprehensive patient care while adhering to HIPAA regulations for confidentiality.
- Provide emotional support and companionship to foster a positive environment for clients living with chronic conditions or disabilities.
Experience
- Prior experience in home health care, assisted living facilities, nursing homes, or long-term care settings is highly desirable.
- Demonstrated proficiency in patient observation, medical terminology, and medication administration.
- Experience working with individuals with developmental disabilities or neurological conditions such as Alzheimer’s or dementia is preferred.
- Familiarity with medical documentation systems including EMR/EHR platforms is advantageous.
- Knowledge of hospice care practices and procedures enhances the ability to deliver compassionate end-of-life support.
- Proven ability to work independently while managing multiple client needs effectively in a home setting.
