Position Summary
Reporting to the Director, Care Coaching/Sr. Manager, Care Coaching, The Manager, Care Coaching provides leadership to shape and implement programs, and processes that support caregivers through remote technology driven coaching. This individual will manage a team of coaches and care advisors to effectively engage, provide education, practical support, and guidance to family caregivers of Medicare Advantage members with a focus on Dual Special Needs Plan (D-SNP) and similar populations. This role ensures high-quality, person-centered coaching that aligns with program goals, health plan contractual requirements, and regulatory expectations resulting in improved caregiver and care recipient outcomes. This includes implementing operational and technical tactics to effectively expand the program and ensure forward compatibility with Careforth’s evolving products and partnerships.
The ideal candidate is an experienced leader who excels in caregiver support models, understands the needs of individuals with complex medical, behavioral, and social needs, and thrives in a mission-driven, fast-paced environment.

The pay range for this position is $73988 - $107,282. The actual wage offered may be lower or higher depending on budget and candidate experience, knowledge, skills, qualifications, and geographic location.

What You Will Do
- Lead, develop and manage a team of care coaches and care advisors delivering telephonic and virtual caregiver support services.
- Provide supervision, case consultation, performance feedback, and skills development. Hire, onboard, train and retain team.
- Support the caregiver journey from the referral management process through ongoing Coaching. Coaches will engage, motivate, and educate family caregivers via phone and technology; may provide direct coaching to caregivers and care recipients as needed.
- Manage daily business operations to include referral workflows, assignment of cases, scheduling, service coverage and prioritization of urgent needs.
- In collaboration with Director and managers, support development and management of caregiver engagement and retention strategies to achieve/exceed census goals
- Review and report on referral activities and trends; work with management to address enrollment challenges
- Facilitate interdisciplinary case reviews and tracking of emerging risk cases; work interdisciplinary on risk mitigation strategies and follow-up
- Responsible for development, implementation, delivery and evaluation of Careforth’s coaching programs
- Lead staff to meet department goals, ensuring key performance indicators and goals are reviewed regularly in support of a high performance / quality of program
- Provides onboarding training to new coaching team members
- May represent Careforth to partners and clients, serving as coaching operations expert
- Support workflow, capacity, and key performance metrics driving to program outcomes; manage departmental resources and goals
- Stay current with operational best practices and tools to support caregiver engagement and behavior change
- Ensure departmental compliance with all regulatory, contracts and accreditation agency standards.
- Coach and mentor coaching team members and identify individual or team training needs
- Collaboration with Coaching Leadership on continuous process improvement activities.
- May have on call responsibilities to support urgent caregiver needs and ensure continuity of care.
- May require flexible hours to support caregivers or staff in varying time zones
- Perform other duties as assigned.
- 3-5 years of progressive management and leadership experience with proven ability to motivate and develop staff
- 3 years of program or solution development experience in various setting that include Medicare and Medicaid programs and/or managed care organization required.
- Demonstrated ability to develop workflows, training documents, communications, and metrics
- Comfort with start-up operations and navigating unchartered territory; views ambiguity as an opportunity and not a deterrent
- Ability to develop, refine and continuously evaluate programs, processes, with limited framework and direction
- Proficiency in common software and web-based applications (MS Office, case management systems)
