Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient health by observing vital signs and reporting any changes to healthcare professionals
- Administer medications and perform catheter care following medical instructions and HIPAA privacy standards
- Support clients with meal preparation, feeding, and nutritional needs to promote overall well-being
- Document medical records accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems, ensuring compliance with medical documentation protocols
- Provide specialized care for individuals with dementia, Alzheimer’s disease, or other memory-related conditions including memory care techniques
- Operate assistive devices such as Hoyer lifts and manage patient transfers safely using proper techniques
- Maintain a clean and safe environment by assisting with light housekeeping tasks like laundry and cleaning in accordance with home & community care standards
- Support hospice or palliative care needs by offering compassionate emotional support and comfort measures
Requirements
- Proven experience in home health, assisted living, nursing homes, or long-term care environments
- Knowledge of medical terminology and procedures related to patient observation and medical documentation
- Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice care or dementia training are a plus
- Ability to work independently while following detailed care plans and instructions from healthcare providers
- Experience working with individuals with disabilities or developmental disabilities is highly valued
- Familiarity with EMR/EHR systems for accurate recordkeeping and documentation
- Strong communication skills to effectively interact with clients, families, and healthcare teams
- Valid driver’s license and reliable transportation are preferred for home visits
