Position Summary

Working as a member of the Access Team to ensure that all individuals who are wanting to access services are able to in a timely manner. This position will focus on assisting clients through opening clients into the Electronic Health Record, working directly with referral resources to assist in client care, compiling required documentation necessary for clients to access services.

Qualifications

• High School Diploma or GED
• Skilled in the use of computer software
• Ability to collaborate with referral resources, clients, and service providers

Responsibilities

1. Work hours prescribed by supervisor and be available to work additional hours when necessary.
2. Oversight of Inquiry system-and registration of client appointments.
3. Compiling and scanning registration documents into electronic health record
4. Documenting any contacts with referral sources or clients within the electronic health record
5. Opening clients into the electronic health record using Inquiry process and transcribing written documents into electronic forms
6. Scheduling client’s appointments and oversight of walk in appointments and waitlist.
7. Coordinate continuity of care between facilities by working to obtain hospital discharge documentation or other relevant collateral information from external sources in advance of client’s appointment.
8. Provide clerical support to Access team members.
9. Document pertinent information daily in the EHR.
10. Serve as a member of the Access team and participate in all team meetings and activities.
11. Attend all scheduled meetings of staff, such as, department meetings, or appropriate committee meetings as designated.
12. Understand and comply with all of the principles established by the Rosecrance Corporate Compliance Program and Code of Conduct.
13. Deliver exceptional customer service consistently to every customer.
14. Serve as a role model for other staff, clients and customers and demonstrate positive guest relations in representing Rosecrance.
15. Assume any additional responsibilities as assigned.

Schedule

• Hours: M-F 8:30-12:30 pm (20 hours/week).
• Shift: 1^st shift

Work Location

Rosecrance Ware Center – Rockford, IL.

• Work Mode: Onsite

Compensation   

• Starting Pay (based on education, experience, and credentials)
  • HS/GED - $19.32 per hour
