Job Summary:
- Reviews and transcribes all medication orders including specialized orders based on approved protocols accurately and timely verifying therapy, correct medication, route, dose and frequency.
- Reviews patient profiles to assure appropriate therapy based on patient specific information that includes at least the following: age, gender, diagnosis, co-morbidities, relevant laboratory values, allergies, height, weight and BSA, pregnancy and lactation status.
Requirements:
- Graduation from an accredited School of Pharmacy. Current active registration and licensure with the State of New Jersey Board of Pharmacy.
- Previous hospital pharmacy experience (2 years) preferred. Previous oncology/infusion centre experience preferred.
- Current active registration and licensure with the State of New Jersey Board of Pharmacy.
