Other advantages of working for us are: Employee Referral Program, our HERO Award recognizing outstanding performance in the line of duty, sabbatical leave after 5 years of service and a 45-minute paid lunch period per shift.POSITION OVERVIEWThe Senior Case Manager (SCM) provides support services for residents living in supportive housing for Transition Aged Youth (TAY) formerly experiencing homelessness. The SCM provides services and case management to his/her caseload requirement of 1:25 ratio and is responsible for service connection, information and referral, crisis intervention, community organizing, groups and activities focused on community building, education, or skill development, and housing retention. Services are provided within the context of harm reduction practices and a Housing First model.ESSENTIAL FUNCTIONSProvide housing/program retention servicesProvide case management services with a 1:25 case ratioCreate and track service plansAssist with activities for community building and empowermentProvide services using harm reduction principlesConduct intakes and ensure that appropriate follow-up and/or referrals are complete and documentedProvide ongoing creative outreach and engagement for supportive counseling, referral, and involvement in building and community eventsProvide crisis intervention, conflict resolution and de-escalation servicesAssist residents in preparation for unit inspections and/or annual re-certificationsParticipate in team meetings, staff meetings, department meetings and organizational work groupsAccompany residents to community-based appointmentsAssist the Resident Services Team Lead with the development and facilitation of groups and community-building activities as neededProvide back-up coverage for other Resident Services staffCreate and track service plansAssist with activities for community building and empowermentProvide services using harm reduction principlesConduct intakes and ensure that appropriate follow-up and/or referrals are complete and documentedProvide ongoing creative outreach and engagement for supportive counseling, referral, and involvement in building and community eventsProvide crisis intervention, conflict resolution and de-escalation servicesAssist residents in preparation for unit inspections and/or annual re-certificationsParticipate in team meetings, staff meetings, department meetings and organizational work groupsAccompany residents to community-based appointmentsAssist the Resident Services Team Lead with the development and facilitation of groups and community-building activities as neededProvide back-up coverage for other Resident Services staffQUALIFICATIONSHigh School diploma or GED requiredBachelor’s degree in a behavioral science (social work, psychology, clinical psychology) or a social service certification (CAADAC, CCAPP, etc.) or equivalent experience preferredThree (3) plus years of experience in the provision of social services or equivalent experienceExperience working with Transition Aged Youth (TAY) highly preferredDemonstrated ability to work with homeless or formerly homeless individuals and those struggling with substance use and mental health issues and disabilitiesAbility to provide effective services to diverse populationsAbility to work collaboratively with referring providers in San FranciscoExperience in crisis intervention, conflict resolution, and counselingExperience in organizing groups, group facilitation and group developmentDemonstrated commitment to help break the cycle of homelessness and poverty, and to empower formerly homeless people by involving them in all aspects of HomeRiseStrong commitment to doing social justice advocacyAdministrative and computer skills (MS Word, Excel)Desired qualification: Fluency in Spanish/Mandarin/Cantonese/Russian a plusPOSITION DETAILSLocation: 374 5th Street, San Francisco, CA Status: Full-Time / Non-Exempt Pay Rate: $31.75 per hour Schedule: Tuesday through Saturday or Sunday through Thursday 9AM - 5PMUnion: This position is under the collective bargaining agreement with SEIU 1021 Reports to: Resident Services Manager PHYSICAL DEMANDS & WORK ENVIRONMENTThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
