Qualifications
- Connecticut RN license
- 2 years of nursing expertise in a Long-Term Care Facility
- Strong supervisory, mentoring abilities, and crisis management skills
- Great organizational, documentation, and compliance knowledge
- Must have strong communication, critical thinking and analytical skills
- Must be an advocate for quality care, staff development and demonstrate a spirit of teamwork
Responsibilities
- Support day-to-day clinical operations for patients needing In-Home Care
- Monitor quality of care delivery and ensure compliance
- Supervise, train, and mentor nurses and caregivers
- Coordinate care planning and communication with physicians, clients/patients, and families
- Ensure clinical documentation and reporting is accurate and compliant
- Client/Patient assessments, supervisory visits, and care plan development and reviews
- Clinical problem solver, and supportive resource for field staff
