In this pivotal role, you'll oversee care management and utilization review functions that ensure every patient receives safe, seamless, and cost-effective care from admission through discharge. You'll guide a talented interdisciplinary team and partner closely with clinical and operational leaders to elevate the patient experience and optimize performance across the continuum of care.
What You'll Do:
- Lead care coordination, discharge planning, and utilization review processes.
- Drive systemwide initiatives that support clinical excellence and efficient resource use.
- Coach and mentor a skilled team of nurses and care management professionals.
- Collaborate with physicians, community partners, and payors to ensure smooth transitions of care.
- Use data and analytics to track trends, improve outcomes, and support organizational goals.
What You'll Bring:
- Bachelor's degree in Nursing (Master's preferred).
- RN with at least 2 years of acute care experience.
- 3 years in care coordination or utilization review leadership at the Director level.
- A visionary, relationship-centered approach to leadership and change management.
