Currently seeking a part-time to full-time Physical Therapist for a position that is available in a mixed caseload: outpatient, home health, and school settings. Key Responsibilities:

Outpatient Setting:

• Evaluate and treat patients with orthopedic, neurological, and post-surgical conditions.

• Develop and implement individualized treatment plans using manual therapy, therapeutic exercises, and modalities.

• Educate patients on home exercise programs and injury prevention strategies.

• Document assessments, interventions, and progress in the electronic medical record (EMR).

Home Health Setting:

• Provide physical therapy services in patients' homes in accordance with physician orders and agency protocols.

• Assess safety, mobility, and function in the home environment.

• Instruct patients and caregivers on exercises, adaptive equipment, and fall prevention.

• Complete timely and accurate home health documentation for insurance and compliance.

School-Based Setting:

• • Deliver physical therapy services to students as outlined in Individualized Education Programs (IEPs).

  • Address gross motor skills, mobility, balance, and access to the school environment.

  • Collaborate with educators, occupational therapists, and other school staff.

  • Participate in IEP meetings and maintain required school-based documentation.

Requirements:

• Doctor of Physical Therapy (DPT) or Master’s degree from an accredited Physical Therapy program.

• Current state license to practice Physical Therapy.

• Experience in an outpatient orthopedic or hospital setting preferred.

• Strong manual therapy and clinical reasoning skills.

• Excellent communication, organization, and documentation abilities.
