Job Summary:

The Occupational Therapist provides skilled therapy services to residents in a Skilled Nursing Facility (SNF), focusing on improving or restoring independence in activities of daily living (ADLs), fine motor function, cognitive skills, and overall safety. The OT evaluates each resident’s functional abilities and develops personalized treatment plans to enhance quality of life, promote independence, and support successful discharge to the least restrictive environment.

Key Responsibilities:

• Perform comprehensive assessments of residents’ physical, cognitive, and psychosocial abilities related to daily function.

• Develop and implement individualized occupational therapy treatment plans based on evaluation findings and physician orders.

• Provide therapeutic interventions to improve self-care skills, upper extremity function, range of motion, coordination, cognition, and visual-perceptual skills.

• Train residents in the use of adaptive equipment and techniques to increase independence in dressing, grooming, toileting, eating, and other ADLs.

• Educate and train caregivers and facility staff in safe handling techniques, environmental modifications, and support strategies.

• Recommend durable medical equipment (DME) and assistive devices as needed.

• Monitor resident progress, modify treatment plans as necessary, and participate in discharge planning.

• Document all evaluations, progress notes, and discharge summaries accurately and in compliance with regulatory and facility standards (e.g., Medicare, Medicaid, HIPAA).

• Adhere to all infection control, safety, and professional practice standards.

Qualifications:

• Master’s or Doctorate degree in Occupational Therapy from an accredited program.

• Current state license to practice as an Occupational Therapist.

• Experience in a SNF or geriatric setting preferred.

• Strong clinical skills and knowledge of evidence-based geriatric rehabilitation.

• Excellent communication, documentation, and team collaboration skills.

• Compassionate, resident-centered care approach.
