Key Duties and Responsibilities
- Assists with the provision of palliative care in accordance with local, state, and federal regulations.
- Orders and interprets diagnostic and therapeutic tests relative to patient’s age-specific needs.
- Implements interventions to support the patient with chronic conditions to decrease recurring hospitalizations.
- Collaborates with care team to manage referrals, follow up calls, and coordination of care.
- Facilitates the patient’s transition within and between health care settings, e.g., admitting, transferring, and discharging patients.
- Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities.
- Monitors the effectiveness of interventions.
- Functions independently to perform age-appropriate history and physical for complex acute, critical, and chronically ill patients.
- Initial home evaluation and monthly visits.
- Attend IDT, and other team meetings as required.
Education and Experience
- Will consider less NP experience with RN hospice and/or homecare experience.
- Minimum of 5 years clinical hospice and palliative care experience. 2 years’ experience in the Nurse Practitioner role preferred.
- Must work well independently and within a team.
- Strong computer skills, including Word, Excel, and PowerPoint.
- At least 2 years NP experience with hospice and/or palliative care experience (either NP or RN).
- Current Advanced Practice Registered Nurse (APRN) Licensure.
- Awareness about UM standards, NCQA requirements, CMS guidelines, Milliman guidelines, and Medicaid/Medicare contracts and benefit systems is helpful.
- Nurse Practitioner certification in Hospice or Palliative Care within 1 year of starting the position.
- Graduate of an accredited School of Nursing.
- Knowledge of clinical standards of care.
- Education and/or Experience: Completion of Nurse Practitioner accredited program.
- Knowledge of long-term care, community resources and cost-effective alternatives regarding patient care delivery systems.
- Minimum of two (2) years clinical experience, desired.
- Exceptional interpersonal skills & superb communication a must.
- At least one (1) year of experience in Hospice or Palliative Care.

Licenses/Certifications
- Must be Board certified.
