Key Duties and Responsibilities
- Collaborate with the attending physician to determine the palliative care needs of the patient
- Determine goals of care with the patient and family/caregiver
- Perform a clinical assessment and determine palliative needs related to:
- Pain and symptom management
- Functional status
- Nutritional status
- Psychosocial needs
- Spiritual needs
- Other care needs
- Discussed findings with the interdisciplinary group and create an individualized plan of care for the patient
- Address the patients palliative care needs
- Monitor the patient progress towards goals and effectiveness of treatment
- Make referrals for care and services as needed
- Consultation services may be provided to patients and family/caregiver, physicians, and other providers regarding recommendations for palliation of symptoms as needed
- Work closely with the members of the interdisciplinary group and oversee the care provided to patients enrolled in the palliative care program
- Facilitate discussion and clarification of patient and family goals of care and communicate these goals to the interdisciplinary group and other healthcare personnel
- Facilitate access to appropriate supportive care services as needed
- Educate patients, family, peers, members of the interdisciplinary group and other healthcare personnel regarding Palliative Care services
- Participate in meetings to discuss and plan patient care
- Assist in the development and implementation of standards of care
- Adhere to standard palliative care practices
Education and Experience
- Graduation from an accredited Medical School.
- Doctorate's degree with a minimum of five (5) years Medical Doctor experience with direct patient care
Essential Skills and Abilities
- Has specialized training in palliative care and/or hospice care, clinical experience in palliative medicine and/or hospice care, or is board-certified or board eligible for certification in Hospice and palliative Medicine
- Maintain controlled substances registration with state and federal authorities
- Pediatrics: has experience and/or training in the developmental stages of pediatric patients
- Participates in ongoing medical education activities related to the medical care of home health/hospice and palliative care patients
- Not excluded from participating in the Medicare program.
- Thrives in an unstructured, start-up environment.
- Self-starter that can work independently and collaboratively, prioritize tasks and has initiative and excitement to take on unfamiliar tasks.
- Advanced knowledge of word processing, graphic presentation and computer software related to specific tasks
- Demonstrated excellent computer and word processing skills with special emphasis on calendaring, presentation, and spreadsheet capabilities
- Working knowledge of company policies, procedures, and operations
- Excellent composition, grammar, and business language skills
- Excellent communication and interpersonal skills with the ability to effectively communicate with all levels of management
- Creative, flexible, well organized, resourceful, and detail-oriented
- Excellent judgment in handling confidential and sensitive information
- Ability to work independently, set priorities and handle multiple tasks with a high level of efficiency

Licenses/Certifications
- Current physician licensure in practicing state, without restriction or subject to any disciplinary or corrective action.
- Certification in a specialist area preferred; (e.g., Internal Medicine, Family Practice, or Hospice and Palliative Care).
