If you’re passionate about patient advocacy, continuity of care, and working in a collaborative environment that values excellence and compassion, we’d love to hear from you.Key ResponsibilitiesAssess and manage patients to identify care needs across medical, behavioral health, and specialty services.Partner with patients and families to establish care goals and coordinate resources to support wellness.Deliver primary care in collaboration with physicians and interdisciplinary teams across post-acute and long-term care settings.Conduct comprehensive assessments, develop care plans, and ensure timely follow-up visits (minimum standard of 260 visits per month).Focus on health management, education, advocacy, and preventive care.Maintain accurate, timely medical documentation using EMR systems.Review and manage medications, labs, and diagnostic tests.Collaborate with Care Coordinators, case managers, and PCPs to improve care quality and efficiency.Participate in interdisciplinary team meetings, peer review, and quality improvement initiatives.Provide rotational on-call coverage per company policy (evenings/weekends reimbursed).Ensure timely completion of medical records, wellness visits, and discharge summaries.QualificationsMaster’s degree in Nursing with emphasis in adult/geriatric care.Active and unrestricted Board Certification and DEA license.State certification as an Adult/Geriatric Nurse Practitioner.Current CPR certification.Prior experience in geriatrics, managed care, or skilled nursing facilities preferred.Skills & AttributesStrong foundation in internal or geriatric medicine.Excellent communication, interpersonal, and patient engagement skills.Ability to work independently while collaborating effectively across teams.Organized, flexible, and detail-oriented with sound clinical judgment.Comfortable in dynamic or start-up healthcare environments.Dedicated to continuous learning, patient advocacy, and care improvement.Core CompetenciesInstills trustCustomer-focusedManages ambiguityCollaborates effectivelyDrives resultsWhat We OfferCompetitive base salary performance bonus.Comprehensive benefits package.Collaborative, mission-driven culture focused on quality care.Opportunities for growth within a fast-expanding healthcare organization.Note:To ensure the health and safety of our workforce, proof of full COVID-19 vaccination is required as a condition of employment, subject to legally recognized accommodations.Company DescriptionCareConnectMD is a provider enabling organization dedicated to caring for frail, elderly, and medically complex populations.
