RESPONSIBILITIES:
- Perform home visits for patients in a home care setting
- Observes, records, and coordinates care with multi-disciplines regarding the patient's treatment and any change in condition
- Teaches, supervises, and counsels the patient and family regarding home therapy procedures and other care needs as appropriate to the patient’s condition
QUALIFICATIONS:
- Current license as an RN in Connecticut
- Valid driver’s license
- Reliable transportation and willing to travel in New London County
- Strong communication and interpersonal skills
- Home health experience including Medicare preferred
