Duties
- Conduct Onboarding in home assessments of clients seeking support services at home
- Create non-medical care plans to include activities of daily living, light housekeeping, personal care, medication reminders, community transportation, respite care, etc.
- Onboard new clients and periodically reassess client's progress with the in-home care services
- Educate patients and their families on non-medical in-home care services and scheduling
- Utilize electronic medical record systems (AxisCare) to accurately document patient information
- Schedule initial caregiver visits
- Pass drug screening
- Valid Driver's License
- Own Reliable Transportation to travel throughout Prosper and surrounding areas
- Proof of Car Insurance
- Social Security Card or Birth Certificate
- Negative TB skin test
- Provide three work related references
