Client Summary
- 32-year-old female
- Diagnoses include:
- Moderate Intellectual Disability (ID)
- Intractable Mixed Seizure Disorder
- Oppositional Defiant Disorder (ODD)
- Obsessive Compulsive Disorder (OCD)
- Verbal with limited vocabulary
- Ambulatory
- Typically sleeps through the night without concerns
- Sleep should not be interrupted, as disruption may trigger seizures
- Seizures are unpredictable and most commonly occur overnight or early morning
- Requires within arm’s reach supervision at all times for safety
- Seizure protocol in place from University of Pennsylvania Neurology
- Provide 1:1 nursing supervision and care
- Take vital signs prior to medication administration
- Administer medications at 12:00 PM and 8:00 PM
- Monitor for seizure activity and follow established seizure protocol
- Evaluate client status after seizure activity
- Administer rescue medication per seizure protocol when required
- Complete seizure logs and incident reports
- Maintain daily nursing documentation
- Assist with activities of daily living (ADLs) including:
- Meal preparation
- Feeding assistance
- Toileting
- Personal care
- Ensure the client’s safety and well-being at all times
- Current Licensed Practical Nurse (LPN) license in [State]
- Experience with seizure disorders preferred
- Experience working with individuals with intellectual/developmental disabilities preferred
- Ability to respond calmly and effectively in emergency situations
- Strong documentation and communication skills
- Compassionate, patient-centered approach to care
- Training will be provided on the client's specific seizure protocol
- Nurse must maintain close supervision and situational awareness at all times
Salary : $38 - $40
