Role Description This full-time, on-site Home Health Aide role is based in New Canaan, CT. The Home Health Aide will assist clients with activities of daily living, including bathing, grooming, dressing, toileting, and mobility support. Responsibilities include providing personal care, light housekeeping, meal preparation, and support with errands as needed. The role involves monitoring clients’ well-being, reporting changes in condition to supervisors, and following individualized care plans. The Home Health Aide will also offer companionship, encourage safe engagement in daily activities, and help maintain a clean, comfortable home environment.

Qualifications
- Demonstrated skills in Home Care and Patient Care, with experience supporting clients in their homes.
- Ability to assist with Activities of Daily Living, including bathing, dressing, toileting, and mobility.
- Strong Communication skills, including clear verbal communication, active listening, and respectful interaction with clients, families, and team members.
- Proficiency in providing Personal Care with sensitivity, privacy, and respect for client preferences.
- Previous experience as a home health aide, personal care aide, or similar caregiving role preferred.
- High school diploma or equivalent; completion of a recognized caregiver or home health aide training program is a plus.
- Ability to follow care plans, document services accurately, and adhere to safety and infection control guidelines.
- Reliability in attendance, punctuality, and maintaining professional boundaries and ethics.
- Physical ability to assist with transfers, stand for extended periods, and perform light housekeeping tasks.
- Valid driver’s license or reliable transportation to client homes in New Canaan, CT and surrounding areas.
