What Success Looks Like in This Role

- Ensure excellent client satisfaction and continuity of care
- Build strong relationships with clients, families, and caregivers
- Proactively resolve service concerns and scheduling challenges
- Maintain high caregiver engagement and retention

Client Care Management
- Conduct client assessments and develop individualized care plans
- Serve as the primary point of contact for clients and their families
- Monitor client satisfaction and proactively address concerns
- Ensure care plans are implemented correctly and updated as needs change
- Perform periodic client check-ins and quality assurance visits
- Collaborate with scheduling to ensure proper caregiver-client matches
- Support caregiver onboarding and training related to client care needs
- Provide ongoing coaching and support to caregivers in the field
- Address caregiver performance concerns and ensure quality standards are met
- Work closely with office leadership to maintain efficient daily operations
- Communicate changes in care needs or schedules to the appropriate team members
- Maintain accurate documentation and compliance with company policies
- Support recruiting and retention efforts for caregivers

- 3 years of experience in home care, healthcare, or service-based operations
- Experience with client services, care coordination, or case management
- Strong communication and conflict resolution skills
- Ability to manage multiple priorities in a fast-paced environment
- Excellent organizational and documentation skills
- Comfortable using scheduling software and office systems
- Bachelor’s degree in healthcare administration, social work, or a related field preferred

Salary : $45,000 - $55,000
