About The Role

Responsibilities:
- Provide evidence-based, patient-centered care to residents in skilled nursing facilities
- Conduct on-site visits and remote patient monitoring to detect abnormalities and prevent hospitalizations
- Collaborate with care teams to develop and implement individualized care plans
- Maintain accurate and timely documentation of patient care and services
- Participate in quality improvement initiatives to enhance patient outcomes and facility ratings
- Active NP or PA license (in the state of practice)
- 1 year of experience in Geriatric care and Documentation review
- Excellent communication, collaboration, and teamwork skills
- Ability to work independently and as part of a remote team
- Annual salary range of $23000 - $25000
