- Build strong relationships with facility staff and patients while maintaining a patient-centered approach
- Collaborate with a multidisciplinary team to provide comprehensive care
- Conduct on-site visits and remote work from home
- Document patient care efficiently with the support of a dedicated scribe team
- Participate in education and training programs to stay up-to-date with industry developments
- Active NP or PA license in the state of practice
- 1 year of experience in Geriatric care and documentation review
- Excellent communication, collaboration, and teamwork skills
