We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to join our team at Carease Health - PA. As a Nurse Practitioner, you will play a vital role in delivering high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

As a Nurse Practitioner at Carease Health, you will be part of a nationwide preventative healthcare company focused on reducing hospitalizations. We prioritize proactive measures for enhanced physical health and well-being, combining Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) into our comprehensive preventative care program. Your success in this role depends on building strong relationships with the facility, maintaining a patient-centered approach, and being a collaborative team player.
Responsibilities
Provide high-quality preventative care to residents in skilled nursing facilities
Conduct on-site visits and remote work from home, supported by a dedicated scribe team for efficient documentation
Build strong relationships with the facility and maintain a patient-centered approach
Collaborate with other healthcare professionals to provide comprehensive care
Participate in education and training sessions to stay up-to-date with industry best practices
Requirements
Active NP or PA license (in the state of practice)
1 year of experience in Geriatric care and documentation review
Excellent communication, collaboration, and teamwork skills
Masters degree in Nursing or related field
