A Registered Nurse whose primary responsibility is to assist the Director of Clinical Services/Administrator with the oversight and management of the Oasis and Plan of Care program. The Registered Nurse may also perform supervision of home health and home care services (Careage HH and Careage@Home) to patients in their place of residence in accordance with physician orders and strives to provide the highest quality of care.
All Skilled nursing and other therapeutic services are furnished under the supervision of the DCS and Clinical Manager. The Clinical Manager is always available during operating hours (or appoints a similarly qualified alternate) and participates in all activities relevant to professional services furnished, including the development of qualifications and assignment of personnel.
Requirements
- Graduate of an approved school of professional nursing or therapy school
- Washington State RN License Required
- BLS- Healthcare is Required
- COS-C certification is preferred. May be obtained at the discretion of the agency if not certified at time of hire.
- Three to five (3-5) years of experience, preferred.
- One (1) year experience in home health, preferred.
- Acceptance of philosophy and goals of Agency.
- Ability to exercise initiative and independent judgment.
- Ability to work with individuals to enlist cooperation of many people to perform/achieve a common goal.
Summary of Responsibilities
- Provides services in accordance with the plan of care.
- Makes the initial evaluation visit and regularly reevaluates the patient’s nursing needs.
- Initiates the plan of care and necessary revisions.
- Provides those services requiring substantial specialized nursing skills.
- Initiates appropriate preventive and rehabilitative nursing procedures.
- Prepares clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner as per Agency policy.
- Coordinates services.
- Informs personnel of changes in the condition and needs of the patient.
- Counsels the patient and family/significant others in meeting nursing and related needs.
- Participates in and presents in-service programs.
- Understands and adheres to established Agency policies and procedures.
- Processes orders and notifies physician of patient needs and changes in condition. Completes certification/recertification orders and discharge summaries.
- Determines the amount and type of nursing needed by each individual patient.
- Refers and coordinates care to Physical Therapist, Speech Language Pathologist, Occupational Therapist and Medical Social Worker those patients requiring their specialized skills.
- Supervises and teaches other nursing personnel in the RN-CM’s team.
- Conducts patient care conferences on patients assigned to his/her care. Participates in peer review and Quality Assessment and Performance Improvement as assigned.
WORKING ENVIRONMENT:
Works indoors in Agency office and patient homes and travels to/from patient homes, if necessary. Primary location of work is in Agency office.
