REGISTERED NURSE (RN) CASE MANAGER

Care United Home Health LLC is seeking a compassionate, experienced, and dependable Registered Nurse (RN) Case Manager to join our growing team.
SCHEDULE COMMITMENT
FULL-TIME: Must be available to work 5 days per week, Monday–Friday, 8:00 AM–4:00 PM.

WHAT YOU'LL DO
- Conduct comprehensive nursing assessments in patients' homes.
- Develop, update, and monitor individualized plans of care.
- Coordinate patient care and communicate with physicians and the clinical team.
- Obtain and follow up on physician orders.
- Educate patients and families regarding diagnoses, medications, treatment plans, and self-care.
- Monitor patient progress and report significant changes in condition.
- Administer medications and monitor patient response to treatment.
- Complete accurate and timely clinical documentation.
- Maintain compliance with agency policies, state regulations, and home health requirements.
- Communicate with patients, families, physicians, therapists, social workers, and other healthcare professionals.
- Provide emotional support and guidance to patients and their families.
- Assist with case management and coordination of services.
HHA CLASSROOM TEACHING
As part of this position, the RN Case Manager will also assist with HHA (Home Health Aide) classroom training and education.
Responsibilities may include:
- Teaching HHA classroom courses and assigned training topics.
- Providing instruction and guidance to HHA students.
- Reviewing course materials and ensuring students understand required skills.
- Preparing students for HHA competency requirements and examinations.
- Maintaining accurate training and attendance records.
- Supporting new HHA employees during training and onboarding.
- Providing hands-on instruction and demonstrations when appropriate.
QUALIFICATIONS – REQUIRED
- Active Indiana Registered Nurse (RN) license.
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited nursing program.
- Current CPR certification.
- Minimum of 2 years of nursing experience.
- Valid driver's license, reliable transportation, and current auto insurance.
- Strong clinical assessment, communication, organizational, and documentation skills.
- Ability to work independently and manage multiple patients and responsibilities.
PREFERRED QUALIFICATIONS
- Home Health or Case Management experience.
- Experience teaching, training, or mentoring healthcare staff.
- Experience with HHA training or caregiver education.
- Experience with electronic health record (EHR) systems.
- OASIS documentation experience.

WORKING CONDITIONS
- Travel throughout the assigned service area to provide patient care in patients' homes.
- Combination of field-based work and occasional office responsibilities.
- HHA classroom instruction and training may be conducted at the agency office.
- Ability to perform routine nursing duties, including standing, walking, bending, and assisting patients as needed.
- Must be comfortable traveling to patients' homes.
- Occasional on-call or weekend coverage may be required based on patient needs.

Pay: $75,000.00 - $88,000.00 per year
