You will provide direct case management services to an adult mental health population. You will spend 50% of your time face-to-face with consumers in their homes and the community and have an opportunity to use various evidence-based modalities (such as CBT, MI, etc.) to aid in the mental health recovery journey of your clients.
What You Will Do
- Facilitate and coordinate psycho-educational services for consumers and their family members on a variety of topics relating to wellness, health promotion, prevention, skill-building, and recovery topics.
- Offer on-site and outreach services (including transport of consumers)
- Fulfill case management services, including coordinating or directly facilitating linkages to services, service coordination, and treatment enrollment assistance.
- Ensure consumers have community support in place to address identified needs.
- Attend routine supervision and team meetings to review cases and discuss program operations.
- Implement evidence-based interventions (CBT, MI, etc.) and assessments to help individuals achieve their treatment goals, link to community resources, and provide inter-agency coordination of treatment.
- Participate in supportive counseling and crisis intervention.
- Participate in the assessment and evaluation of consumers at intake/admissions, during ongoing recovery/service delivery, and at discharge.
- Conduct case management intake assessments and formulate plans of care.
- Perform reviews of progress and evaluate the effectiveness of goals.
- Work with administrative staff to ensure that all services are documented based on billing and regulatory standards.
- Master's Degree in a Mental Health-related field OR
- Bachelor's Degree in a Mental Health-related field, plus one (1) year post-BA experience in the Mental Health field
- Valid driver's license insurable by agency's automobile insurance
