Job Summary

The role requires maintaining accurate and timely records for each case, collaborating with internal and external partners, and supporting departmental goals within prescribed timeframes.
What You Will Do
- Clinically assess clients at intake and/or admission, during ongoing treatment, and at discharge/transfer
- Respond to clients for crisis intervention
- Formulate clinical psychosocial treatment plans, with relevant goals, measurable objectives that serve as indexes for evaluating treatment progress, and realistic time frames for achievement
- Provide clinical care coordination to support client's treatment goals
- Plan, communicates, and document discharge/aftercare/transfer plans for clients completing treatment and/or accessing other services in the continuum
- Maintain ethical boundaries with clients and staff
- Maintain client confidentiality as set for by CFR 42 part 2
- Submit documentation to Supervisor daily for any/all sessions held with clients on that day only if staff is provisionally license.
- Competent in NJSAMS
- Encourage and supports clients' participation in AA/NA, SMART Recovery
- Follow all Electronic Health Records (EHR) policies
- Conduct and monitor urine screens
- Master's degree in a behavioral health discipline from an accredited college or university
- 1-2 years of clinical experience in the mental health field, including but not limited to internships, community-based services, case management, etc.
- Provisional/Professional Licensure to practice counseling/clinical social work (LAC, LSW, LAMFT, LPC, LCSW, LMFT, LCADC)
- A valid driver's license and you must be insurable by the agency's automobile insurance
