Qualifications
- Strong Communication skills, including clear verbal and written interactions with patients, families, and healthcare professionals.
- Demonstrated Customer Service skills with a compassionate, patient-centered approach in sensitive end-of-life situations.
- Experience with Case Management or care coordination, including organizing referrals, tracking admissions, and collaborating with interdisciplinary teams.
- Working knowledge of Medical Terminology and basic clinical documentation relevant to hospice or healthcare settings.
- Familiarity with Insurance processes, including eligibility verification, authorization, and coordination with payers.
- Proficiency with electronic health records (EHR) and standard office software (e.g., email, spreadsheets, scheduling tools).
- High attention to detail, strong organizational skills, and the ability to manage multiple referrals and tasks simultaneously.
- Ability to maintain confidentiality and comply with HIPAA and organizational policies.
- Previous experience in hospice, home health, or healthcare intake/registration is preferred.
- High school diploma or equivalent required; additional training or coursework in healthcare administration or related field is a plus.
