Duties & Responsibilities
- Conduct thorough assessments of patients' physical, emotional, and spiritual needs.
- Develop individualized care plans based on assessments, patients' wishes, and medical directives.
- Collaborate with interdisciplinary teams to ensure care plans address comprehensive needs.
- Administer medications and treatments to manage pain and alleviate distressing symptoms.
- Monitor and evaluate the effectiveness of interventions to ensure patient comfort.
- Provide emotional support to patients and their families, addressing fears, concerns, and emotional distress.
- Facilitate open and honest conversations about patients' feelings, wishes, and end-of-life goals.
- Educate patients' families about the dying process and what to expect in the final stages.
- Provide guidance on caregiving techniques, emotional support, and coping strategies.
- Maintain accurate and up-to-date electronic health records (EHR) that reflect patient assessments, interventions, and outcomes.
- Ensure adherence to ethical guidelines and legal regulations in end-of-life care.
