The incumbent conducts assessments, develops treatment plans, implements interventions, and collaborating with patients, families, and other healthcare professionals to achieve optimal outcomes.
Duties & Responsibilities
- Create and implement evidence-based treatment plans that are tailored to the needs of the patients, with an emphasis on improving the Patients' cognitive, fine motor, sensory, and daily living skills.
- Collaborate with patients, their families, and the multidisciplinary team to establish personalized treatment goals and plans
- Utilizes evidence based practice to select OT treatment appropriate to the needs of the patient
- Provides role as patient advocate through his/ /her own role modeling of standards of performance and by communicating and meeting patients needs for service
- Routinely monitor and evaluate patients' progress and adjust treatment plans as needed to ensure optimal outcomes
- Educate patients, families, and caregivers about strategies to facilitate continued progress and maintain functional gains at home and in various external settings.
- Maintain accurate and up-to-date documentation of assessments, treatment plans, progress notes, and other relevant records in accordance with regulatory guidelines.
- Adhere to ethical standards and professional guidelines in all aspects of practice.
