Duties
- Supervise and support home health care aides, ensuring they deliver exceptional patient care aligned with agency standards and protocols
- Coordinate schedules, assignments, and patient visits to optimize service delivery and efficiency
- Monitor patient progress through direct observation, medical documentation, and communication with healthcare teams
- Ensure compliance with HIPAA regulations and maintain confidentiality of patient records and medical information
- Provide training and mentorship on essential caregiving skills such as assisting with ADLs (Activities of Daily Living), medication administration, catheter care, hospice care, dementia care, Alzheimer's care, memory care, and use of EMR (Electronic Medical Records) systems
- Conduct regular assessments of aide performance, providing feedback and coaching to promote continuous improvement
- Oversee the proper use of medical equipment such as Hoyer lifts, vital signs monitoring devices, and other assistive tools
- Maintain accurate documentation of patient interactions, medical records, and care plans in electronic health record (EHR) systems
- Promote a safe environment by adhering to infection control protocols and conducting safety checks during visits
- Collaborate with healthcare professionals to develop personalized care plans for clients with complex needs including long-term disabilities or developmental disabilities
Skills
- Strong knowledge of medical terminology, patient observation techniques, and home health procedures
- Proven experience in assisted living, nursing homes, or home & community care environments
- Expertise in working with individuals with disabilities, dementia, Alzheimer's disease, or other memory impairments
- Excellent leadership skills with the ability to motivate and supervise a team of caregiving professionals
- Proficiency in EMR/EHR systems for documentation and reporting purposes
- Ability to assist with ADLs such as bathing, dressing, grooming, meal preparation, laundry, cleaning, and mobility support using equipment like Hoyer lifts
- Knowledge of hospice care principles and medication administration protocols
- CPR certification along with training in infection control practices and HIPAA compliance
- Effective communication skills for coordinating with clients’ families and healthcare teams
- Experience in managing patient care plans for diverse populations including seniors and individuals with developmental disabilities
