The role ensures all referrals, documentation, timelines, and communications are completed accurately, timely, and in compliance with DDSN Intake Standards using the Therap system.

The Eligibility Intake Service Provider (ISP) is responsible for completing the intake and eligibility determination process for individuals applying for services through the South Carolina Department of Disabilities and Special Needs (DDSN). The role ensures all referrals, documentation, timelines, and communications are completed accurately, timely, and in compliance with DDSN Intake Standards using the Therap system.
Essential Duties and Responsibilities
- Initiates contact with applicants or their representatives and maintains communication throughout the intake process.
- Collects, reviews, and uploads required medical, educational, developmental, and psychological documentation based on eligibility category.
- Completes required administrative forms.
- Documents all contacts and activities accurately and within required timelines.
- Submits finalized eligibility packets for DDSN review and resolves returned items.
- Coordinates evaluations within established deadlines.
- Refers applicants for additional services when ongoing needs are identified.
- Maintains confidentiality and compliance.
- Knowledge of intellectual, developmental, physical, and early intervention disabilities.
- Ability to interpret medical, educational, and psychological records.
- Proficiency with electronic case management systems (preferred, not required).
- Strong organizational, communication, and time-management skills.
- Attention to detail and regulatory compliance.
- Experience in intake, eligibility, or disability services preferred.
- High school diploma or GED equivalent
- Proficiency in Microsoft Office and Adobe Acrobat
- Remote work environment.
- Frequent computer use and detailed electronic documentation.
- Regular interaction with applicants, families, providers, and DDSN staff.
- Timeliness and accuracy of intake and eligibility submissions.
- Compliance with DDSN timelines and documentation standards.
- Quality and completeness of eligibility packets.
Salary : $45,000 - $51,000
