What You’ll Do & Key Responsibilities
- Support residents and families with emotional, psychosocial, and adjustment needs during their stay.
- Coordinate care planning and collaborate with interdisciplinary teams to support resident goals and well-being.
- Assist with admissions, discharges, and transition planning to ensure continuity of care.
- Connect residents and families with community resources, support services, and financial assistance programs as appropriate.
- Maintain accurate documentation and ensure compliance with federal, state, and facility requirements.
- Bachelor’s degree in social work, Gerontology, or a related Health and Human Services field.
- Ability to obtain or maintain a Licensed Social Worker (LSW) credential.
- Strong interpersonal and communication skills with the ability to build positive relationships and communicate effectively.
- Previous experience in healthcare, long-term care, or skilled nursing is preferred but not required.
