What it takes to become an LPN with Care Initiatives: Valid Licensed Practical Nurse (LPN) license in good standing. License must be valid within the state of Iowa, or ability to activate. Current, valid CPR Certification. Documented completion of State-approved LPN Supervisory Course or ability to obtain within first 90-days of employment.
