Rapidly expanding aging service provider seeks full time Case Managers/Care Coordinators for its Aging Services program in Coles and Edgar counties.
Duties and Responsibilities:
- Performing in-home comprehensive assessments,
- Performing hospital and LTC facility based assessments, including week-end rotation
- Developing, monitoring and revising person centered plans of care
- Providing linkage and referral to other social service organizations
- Follow up activities, etc.
- Adherence to program timeframes and policies a must.
Qualifications and Skills:
- Candidate should possess a bachelor's degree in social science or health field or be a registered nurse. Formal work experience with the 60 population can be substituted for bachelor's degree. One year of experience for one year of college, up to four years.
- Master's degree preferred
- Positions work out of the Charleston office
- Extensive travel and dependable transportation with vehicle insurance required
- Valid Driver’s license
- Computer literacy
- Strong organizational skills and the ability to work independently
- Bilingual candidates preferred
