The Personal Habilitation Worker's (PAB) / Community Learning Services-Individual (CLS-I) purpose is to facilitate the participant's involvement in the home and community setting and maximize their independence. Services will be performed in various environments.
· Minimum Education: High School Diploma or GED
· Minimum Field-of-Expertise: One year of experience working with developmental/intellectual population. A good work ethic and strong interest to work with special needs population and their families/caregivers. Must be able to coordinate participant activities and work collaboratively with the Service Supervisor, and DOH, ID/DD Case Manager, and other members of a participant's team.
Duties and Responsibilities:
· Implements evidence-based individualized services under the direction and supervision of the Service Supervisor
· Adheres to agency policies and procedures
· Able to follow a written plan that details the skills to be taught, the instructional strategies to be utilized across domains and the data management strategies to be implemented
· Participates in agency training and supervision as required
· Collects program and participant data as needed; shows integrity with the quality of work being turned in
· Fosters good relationships among care providers; maintains dialogue in interaction with other professionals to identify and resolve existing problems
· Participates/contributes to meetings pertinent to area of responsibility; promotes quality participant care through a spirit of cooperation with other staff, caregivers, and other circle of support team members
· Is consistently on time and available to provide direct services where the participant needs the service to be provided
· Demonstrates self-evaluation skills and takes responsibility for and appropriate action to remedy or reduce limitations; independently seeks formal and informal professional development activities to ensure the highest standard of knowledge and skill.
· Must have reliable transportation, be able to transport participants to and from their home and/or to and from their community location of choice and home.
