Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and observe patients’ health status, including vital signs and overall condition, ensuring timely reporting of any changes
- Administer medications accurately following prescribed schedules and medical documentation protocols
- Support patients with medical procedures such as catheter care, wound care, and other specialized treatments
- Maintain detailed records using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems, ensuring compliance with healthcare regulations like HIPAA
- Provide companionship and emotional support to clients, especially those with dementia or Alzheimer’s disease
- Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe living environment
- Use patient transfer methods safely, including Hoyer lifts or other mobility aids, to prevent injury for both client and caregiver
Qualifications
- Proven experience in home healthcare, assisted living, nursing homes, or long-term care environments
- Strong knowledge of medical terminology, medication administration, and vital signs monitoring
- Experience working with individuals with developmental disabilities or geriatrics, including dementia and Alzheimer’s care
- Certifications in First Aid, Basic Life Support (BLS), CPR, and hospice or end-of-life care preferred
- Familiarity with EMR/EHR systems and medical documentation standards
- Ability to lift heavy objects safely and perform patient transfers using appropriate techniques and equipment
- Excellent communication skills for client interaction and team collaboration
- Knowledge of home health care regulations and maintaining patient confidentiality under HIPAA standards
