Precise pay rates are determined through consideration of one or more of the following: education, training, relevant experience and work location. There are some cases in which the pay rate could exceed the range listed on this job post. Pay Range
$38—$53 USD
Benefits are offered to employees that are scheduled to work 20 hours/week, which include a generous earned time (vacation days) program, tuition reimbursement, scholarship programs, student loan paydown program, two retirement plans, in addition to medical/dental/vision/life/disability insurance, and so much more!

As a vital member of our interdisciplinary team, you'll work under the supervision of a Clinical Manager to coordinate all aspects of patient care—ensuring individualized, compassionate support that maximizes quality of life.
Key Responsibilities:
- Conduct ongoing, holistic assessments of patients and families
- Develop and manage comprehensive care plans in coordination with the care team
- Facilitate interdisciplinary case conferences and care planning discussions
- Collaborate with physicians, hospice staff, volunteers, and external agencies to ensure optimal support
- Advocate for patients and families, addressing physical, emotional, and psychosocial needs
- Active Massachusetts RN license in good standing
- Associate's degree in Nursing required; BSN or equivalent preferred
- Proven ability to provide care with compassion and cultural sensitivity in diverse home or facility settings
- Strong interpersonal skills and the ability to thrive in a collaborative team environment
- Flexibility, self-direction, and sound clinical judgment
- Valid driver's license and access to an insured, reliable vehicle
