Per Diem
Summary
The Per Diem Physical Therapist provides skilled hospice care services to adult and geriatric patients across a variety of settings, including private homes, assisted living communities, and inpatient facilities.
As a key member of the interdisciplinary care team, the Physical Therapist delivers patient- and family-centered care that promotes comfort, safety, mobility, and dignity at end of life. This role supports patients in remaining in their home setting whenever possible while receiving high-quality, compassionate care.
Qualifications
- Minimum of one year of clinical experience as a licensed Physical Therapist required.
- Graduate of an accredited physical therapy program.
- Home health or hospice experience strongly preferred; or an equivalent combination of education and experience that provides the necessary skills to perform the role.
- Ability to work independently and manage time effectively in a field-based environment.
- Strong clinical assessment, problem-solving, and organizational skills.
- Excellent interpersonal and communication skills.
- Proficiency with electronic medical records (EMR) systems preferred.
- Valid driver's license and reliable transportation required.
