Job Summary: Provides social services to patients facing substantial and often challenging medical
issues. Individuals recovering from illness or trauma may become psychologically, emotionally, and
socially destabilized; it is the duty of a medical social worker to provide support for these patients. They
help enhance the interaction between patients and healthcare providers, thereby ensuring that the best
interests of the patient are being met.
Reports to: Administrator/Director of Patient Care Services
Essential Functions:
1. Investigate child abuse or neglect cases and take authorized protective action when necessary.
2. Advocate for clients or patients to resolve crises.
3. Collaborate with other professionals to evaluate patient’s medical or physical condition and to
assess client needs.
4. Refer patient, client, or family to community resources to assist in recovery from mental or
physical illness and to provide access to services such as financial assistance, legal aid, housing,
job placement or education.
5. Counsel clients and patients in individual and group sessions to help them overcome
dependencies, recover from illness, and adjust to life.
6. Utilize consultation data and social work experience to plan and coordinate client or patient care
and rehabilitation, following through to ensure service efficacy.
7. Plan discharge from care facility to home or other care facility.
8. Organize support groups or counsel family members to assist them in understanding, dealing
with, and supporting the client or patient.
9. Modify treatment plans to comply with changes in clients’ status.
10. Monitor, evaluate, and record client progress according to measurable goals described in
treatment and care plan.
11. Identify environmental impediments to client or patient progress through interviews and review of
patient records.
12. Supervise and direct other workers providing services to clients or patients.
13. Oversee Medicaid- and Medicare-related paperwork and recordkeeping in hospitals.
14. Plan and conduct programs to combat social problems, prevent substance abuse, or improve
community health and counseling services.
15. Develop or advise on social policy and assist in community development.
16. Maintain a positive and respectful attitude and demeanor.
Knowledge, Skill and Experience:
1. Graduate of a bachelor’s program in social work accredited by the Council on Social
Work Education.
2. Minimum of one (1) year's experience in health care.
3. Experience in a home health care preferred.
4. Demonstrates good verbal and written communication, and organization skills.
5. Skills Required: Must be able to maintain strict confidentiality with regard to protected and sensitive
information. Must be able to solve problems independently and confidently by applying analytical and
logical thinking.

Essential Physical Tasks: While performing the duties of this job, the employee is frequently required to
