Job Summary:
The Hospice Social Worker - MSW provides compassionate and comprehensive social work services to patients and families facing end-of-life care. They support individuals and their loved ones through emotional, financial, and practical challenges that come with terminal illness.
Responsibilities and Duties:
1. Conduct psychosocial assessments to identify patient and family needs.
2. Develop and implement care plans that address emotional, social, and financial concerns.
3. Provide counseling and support to patients and families coping with grief and loss.
4. Collaborate with interdisciplinary team members to ensure holistic care for patients.
5. Assist with advanced care planning and end-of-life decision-making.
6. Coordinate community resources and referrals to support ongoing care needs.
7. Advocate for patients' rights and ensure their wishes are respected.
8. Maintain accurate and timely documentation of patient interactions.
9. Participate in ongoing education and training to enhance skills and knowledge.
10. Provide bereavement support to families after the loss of a loved one.
Qualifications and Skills:
1. Master's degree in Social Work (MSW) from an accredited program.
2. Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) preferred.
3. Minimum of 2 years of experience working in hospice or palliative care.
4. Knowledge of ethical standards and best practices in end-of-life care.
5. Strong communication and interpersonal skills.
6. Ability to provide emotional support and empathy to patients and families.
7. Proficient in conducting assessments and developing care plans.
8. Understanding of grief and loss processes.
9. Ability to work collaboratively in a multidisciplinary team.
10. Proficiency in electronic medical records and documentation.
