Job Summary:
We are seeking a dedicated Case Manager to oversee and coordinate the care of individuals in need of social services. The ideal candidate will be compassionate, organized, and possess strong communication skills. In this role, you will work closely with clients, their families, and other service providers to ensure that all needs are being met and appropriate resources are being accessed.
Responsibilities and Duties:
- Conduct comprehensive assessments to determine client needs and develop individualized care plans
- Coordinate services and resources for clients, such as medical care, housing, and job placement
- Monitor client progress and make adjustments to care plans as needed
- Provide advocacy for clients within the healthcare and social services systems
- Collaborate with other professionals, such as doctors, therapists, and social workers, to ensure a holistic approach to care
- Maintain accurate and up-to-date documentation of client interactions and progress
Qualifications and Skills:
- Bachelor's degree in social work, psychology, or a related field
- Previous experience in case management or a related field preferred
- Strong interpersonal and communication skills
- Ability to work independently and as part of a team
- Knowledge of local resources and services for individuals in need
