Care Continuity is looking to hire a full-time, remote, work from home Patient Support Center Care Navigator to work Monday - Friday daytime hours. Full-time employees are eligible for excellent benefits, including insurance benefits stipend, paid time off (PTO), holiday pay, and 401k matching. This position will be responsible for contacting patients that were recently discharged from the hospital and offering them assistance in scheduling their recommended follow-up appointments. They will also be calling physician offices to schedule follow-up appointments for patients that do accept our services. Our ideal candidate will enjoy working independently to complete daily assignments while supporting their team's workload where the ultimate goal is caring for patients. - Outbound and inbound calls to and from patients/provider offices
- Working on the computer to read charts, fax records, look up insurance coverage, and make detailed notes regarding patient scheduling preferences
- Minimum 18 years old with HS diploma or equivalent
- Preferred at least 1 year of college completed
- Comfortable troubleshooting basic computer issues
- Able to navigate comfortably between multiple windows/computer applications
- Good written and oral communication skills
- Self-motivated and responsible
