Build a career helping families find the right senior living options

Care Connect helps older adults and their families navigate senior living decisions. We are hiring for two part-time associate openings across our sales, care navigation, and operations recruiting tracks, with the goal of developing successful hires into long-term Care Coordinators.

This opportunity is ideal for someone who enjoys talking with people, building professional relationships, keeping work organized, and learning a service-focused sales process.

You will begin by supporting experienced team members with referral outreach, tour scheduling, case follow-up, and administrative coordination. As you demonstrate readiness, you will receive training and take on more substantive phone conversations, Medicaid-related placement support, and increasingly complex coordination assignments.

Our target is to evaluate readiness for advancement into a Care Coordinator role around month nine. Advancement depends on demonstrated skills, performance, and business needs; it is not automatic or guaranteed.

What you will do

Support referral outreach and business development

  • Make outbound calls to assigned senior living communities, skilled nursing facilities, and other prospective referral partners.
  • Identify decision-makers and verify names, titles, phone numbers, and email addresses.
  • Send approved introductions, maintain follow-up queues, and help develop relationships in new and existing markets.
  • Document outreach and next steps in HubSpot.
  • Help staff informational tables, community events, and networking activities.
  • Prepare outreach materials and follow up with contacts after events.

Coordinate tours and keep cases moving

  • Schedule tours between families, community representatives, and the assigned Care Connect advisor.
  • Send calendar invitations, confirmations, and reminders; communicate changes and reschedule when needed.
  • Follow up with the advisor after tours and document next steps.
  • Send and track intake forms and outstanding documentation.
  • Provide approved case updates to referral partners, including social workers, case managers, and discharge planners.
  • Identify overdue tasks and bring time-sensitive concerns to the appropriate team member.

Tour responsibilities initially focus on scheduling and follow-up. Participation in tours or broader consultations may be introduced after training and approval.

Learn care navigation and Medicaid placement support

  • Contact facilities to gather current information about admissions requirements, reported availability, payer acceptance, and next steps.
  • Support Medicaid-related placement cases through facility outreach, documentation tracking, and follow-up with families and authorized professionals.
  • After training, conduct assigned information-gathering calls using established workflows.
  • Help resolve logistical and communication barriers in more complex cases under supervision.
  • Recognize questions or concerns that require an experienced advisor’s involvement.

Maintain accurate records and support daily operations

  • Update HubSpot with community information, contacts, tags, communications, and follow-up tasks.
  • Correct or flag duplicate, incomplete, and outdated records.
  • Verify and document move-in dates and monthly rent plus care charges, recording standard charges separately from discounts.
  • Maintain calendars, prospect lists, task lists, and sales tracking materials.
  • Schedule approved social media content and respond to routine comments or inquiries using company guidance.
  • Route sensitive messages, complaints, and case-specific questions to the appropriate team member.
  • Assist with other related sales, marketing, care coordination, and administrative assignments as needed.

  • Who will do well here

    • Someone comfortable making outbound calls and initiating professional conversations.
    • A reliable communicator who follows through without repeated reminders.
    • Someone organized enough to manage several cases, contacts, and deadlines.
    • A coachable person who can accept feedback and apply it.
    • Someone comfortable with email, calendars, spreadsheets, and learning a CRM.
    • A person who handles confidential information carefully and knows when to ask for help.
    • Someone interested in a long-term career combining relationship development, service, and care coordination.

Experience in sales, customer service, hospitality, healthcare, senior living, or administration is helpful but not required. Students, recent graduates, and career changers are welcome; current college enrollment is not required.

    Training and professional judgment

Responsibility will grow with training and demonstrated competence. Associates must work within assigned authority, protect confidential information, and escalate clinical, legal, eligibility, or complex placement questions.

Associates do not independently make clinical assessments, determine Medicaid eligibility, negotiate contracts, or guarantee admissions, pricing, availability, or placement outcomes.

    Schedule and growth

    • 20–30 hours per week, primarily during weekday business hours.
    • A consistent schedule established around business needs and agreed availability.
    • Primarily remote work, with local travel for assigned events and training.
    • Paid training and direct coaching from company leadership and experienced team members.
    • A development path toward long-term Care Coordinator employment, with a target readiness review around month nine.
