Responsibilities:

Work collaboratively with our interdisciplinary team to develop and implement personalized care plans
Requirements:
Current certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA)
Ability to lift, bend, and move safely in a fast-paced environment
Strong communication and interpersonal skills
Ability to work independently and as part of a team
